I declare that I am physically fit to participate and of legal age and competent to sign and agree that a benefit is realized by me as a participant in the Barrhaven Mixed Slo-Pitch League (hereinafter called "BMSPL") For myself, my heirs, executors, administrators, successors and assigns, I hereby release from liability and agree to indemnify and hold blameless the promoters, all other participants, officials, sponsors, advertisers, league executive, owners and/or operators of the premises and facilities (including without limitation BMSPL), and each of them and any of their agents, employees or representatives or otherwise, during my participation and/or attendance at any league events.
In addition, I agree to, at all times, inspect equipment, playing fields, premises and event related facilities for possible risk and determine for myself that conditions are acceptable for me to commence or continue participation and/or attendance.
I also agree my participation and/or attendance, commencing or continuing, indicates acceptance of equipment, fields, premises, et al, and I knowingly and freely assume all risks and full responsibility for my participation and/or attendance, commencing or continuing AND I hereby grant BMSPL and their agents the irrevocable right to use my name, face or other likeness for any and all purposes.
In the event of a non-minor injury, I hereby commit to following the injury reporting process as laid out at bmspl.com in the Rules section; I will also email [email protected] to notify the commissioners within 48 hours of the injury occurring.
I UNDERSTAND THAT AS A PLAYER IN THE BMSPL, IT IS MY RESPONSIBILITY TO REGISTER AND PAY FOR PLAYER INSURANCE COVERAGE TO PROTECT MYSELF AND OTHER LEAGUE MEMBERS IN CASE OF INJURY.
**Must be 18 years of age to sign "Release & Waiver"; for safety reasons, no consideration will be given to anyone under 18 years of age without a release signed by their parent or guardian
Teams & Players are required to adhere to all city by-laws, including as it relates to alcohol & smoking on city property.
No alcohol or cannabis is allowed on the fields (or in the dugout). Intoxicated players will be removed from participation. Individual or team infraction may result in immediate dismissal from league. Player(s) will lose all league fees. Parking lot beer consumption is not permitted as enforced via by-laws.
I am aware that some games are photographed or videotaped for the sole purpose of the League and these images can be used on the BMSPL web site, flyers or posters.

Conduct of Members
BMSPL is committed to providing an environment in which all Members are treated fairly and with respect. Players agree to the terms of membership and agree to abide by the Code of Conduct outlined below.
No player shall:
Cause any form of personal abuse, including verbal, physical and emotional abuse towards other players, officials, host personnel, spectators, sponsors and volunteers.
Refuse to abide by an official's decision, threaten an official, or at any time lay a hand upon, push, shove, strike, or make incidental physical contact with the body or kick dirt or other matter on the official or attempt to cause physical contact when disputing a decision. This includes causing or attempting to cause physical contact with another participant at the tournament.
Make any disparaging remarks or take aggressive actions toward players, officials, tournament personnel, BMSPL, spectators, sponsors, volunteers and others.
Use any form of profane, vulgar, obscene or abusive language or gestures, whether it is directed at an official, opponent, playing colleague, team official, tournament official, BMSPL personnel or spectator.
Use intimidating or threatening behavior towards officials, opponents, playing colleagues, team officials, tournament personnel, BMSPL personnel, sponsors, volunteers.
Throw bats, gloves, benches or other equipment in a forceful manner or cause the destruction of property belonging to BMSPL or others.
Discuss publicly with participants and spectators in a derogatory or abusive manner any decision by the officials or any opinions of players.
Use any intoxicating substances during the course of the game, or appear to be in an intoxicated condition while participating in a game.
Use any improper and unapproved equipment.
Give false or misleading information or fail to cooperate with a legitimate request from a BMSPL official in the discharge of his or her official duties.
Players who, at the Commissioners discretion, have failed to adhere to the points of conduct outlined or members who engage in any other behavior that may be considered detrimental to sponsors, BMSPL or its players may have penalties and/or suspensions rendered against them up to and including permanent dismissal from the league. Player(s) will lose all league fees at the discretion of BMSPL Commissioners.
